Please bring all insurance information, including insurance cards, when you come to any location for an appointment. Many medical insurance companies and HMOs require pre-authorization for medical services. Please consult with your insurance company or primary care physician to obtain a pre-authorization for services, if required, prior to the date of your appointment.
We accept checks and major credit cards for payment of services. Co-payments and payment of non-covered services are required at the time of your visit.

A Flexible Spending Account (FSA), Health Reimbursement Account (HRA), or Health Savings Account (HSA) is an employer-sponsored account that makes it possible for employees to designate pre-tax dollars for future medical expenses. These medical savings accounts are an excellent way for our patients to pay for their vision care, and funds from these accounts may be applied to treatments not covered by medical insurance.
We will check your vision benefit eligibility and make every attempt to help you understand and make the most of your benefits. It is important to understand that things can change between the eligibility check and the actual payment of the claim. For example, changes in insurance, lapse of time, new benefit years, claims that arrive to the payer after eligibility was checked, and purchases made between checking eligibility and placing an order for glasses or contacts. Charges not covered by your insurance company or benefit plan, as well as any co-payments, deductibles, and coinsurance amounts, are your responsibility.
